US Renal Data System 2020 Annual Data Report

US Renal Data System 2020 Annual Data Report shows 30-day hospital readmission rates of chronic
kidney disease (CKD) patients in comparison to other populations. For patients over 66 there was a
16.6% increase in readmission for non-CKD patients. There was a 23.% increase in CKD patients and a
31.1% increase in patients receiving dialysis. 1 The cost of readmission places a financial burden on CMS.
It is estimated in 2015 all readmissions cost Medicare more than $15 million. 2
While the ESRD PPS final rule looks to put a pause on Standardized Hospitalization Ration (SHR) and
Standardized Readmission Ration (SRR) it is important to understand the impact of these events and
ways to improve transitions of care. One of the most common challenges that impact these events are
capturing of information. Did the patient have an acute episode requiring hospitalization? Was there
communication from the hospital to the dialysis unit to assure coordination of care occurred? Were
orders or plans of care followed during the transition from acute to chronic phase?
