The Future of Kidney Care

The CDC estimates that more than 37 million Americans are living with chronic kidney disease. The vast majority don’t even know it because there are typically very few or no symptoms at all. However, early detection can be the difference between life and death.  As a result, our health care system has begun to shift from a fee-for-service model to a value-based model. Let’s explore the two models, and how the shift can be beneficial to the future of kidney care.

The Future of Kidney Care

Fee-for-service Model

A fee-for-service model includes payments coming from the Centers for Medicare & Medicaid Services (CMS) or private plans. Therefore, with this model, payments are made based on a fee for a specific activity. Subsequently, instead of looking at the whole patient, the fee-for-service model focuses on episodic, specific problem-oriented care. The traditional fee-for-service system is fragmented and results in misaligned values and incentives across the healthcare system.

Value-Based Model

The value-based model has a reimbursement system and focuses on achieving better clinical outcomes for patients. This is all done while lowering overall system costs. The valued-based system encourages a more holistic, integrated look at patient care versus episodic care. This health/kidney care model promotes patient-centric care which focuses on preventative care.

Early Intervention

When it comes to kidney care, the value-based model helps identify patients with chronic kidney disease earlier. Of course, this is easier said than done. That is to say, it is achievable with better communication across health systems and points of care. To achieve a more integrated approach, there must be a removal of barriers to this type of care. This includes driving better interoperability between electronic health records (EHRs), finding ways to build the infrastructure to support social, geographic, and behavioral determinants, and having resources in place to assist those who need them at the point of care. To achieve this integrated care approach, incentives must be in alignment.

Kidney Care Technology

Technology can also assist with the navigation of the complex kidney care environment. Modern kidney care technology is able to drive integration between various touchpoints within the system. This integration and community approach will also result in a better patient experience and promote patient compliance by default.

The ability to shift the paradigm of patient care already exists. The next step in effecting change is all about execution.
