Et cetera, previously abbreviated as ETC., and left hanging at the end of a long list, has a new meaning. The new ETC payment models (End Stage Renal Treatment Choices Models,) are moving forward. Impacting small and large dialysis organizations, these models are designed to move patients to alternative treatment options such as home therapy and kidney transplantation. There are limited exclusions to the models, making a facility’s current position perhaps difficult to understand, as we anxiously await the publication of the facility’s baseline. How can your facility prepare for the change?
Understanding patient criteria for inclusion and exclusion is a great way to begin your preparation. Reviewing the list of patients in skilled nursing facilities, insurance information, dementia diagnosis, barriers to transplantation and home therapy for specific patients are most likely all on your holiday “to-do list”.
Visonex created a client workgroup designed to assist clients in developing tools that would be beneficial within the facility to review such information. Additionally, the team created a Calendar Year 2020 ETC Checklist to begin reviewing and projecting targets moving forward.
Did your facility miss the mandate? While that may give you a little time, just remember these models were designed to expand based on the success in both patient outcomes and decreasing cost of care. Now is a great time for all facilities to evaluate their current referral base. Engage providers, communities, and encourage partnerships with practitioners in your area. The National Kidney Foundation notes that 1 in 3 American adults are at risk for CKD (Chronic Kidney Disease) and it is the 9th leading cause of death in the United States. With statistics like these, understanding the cost of care associated with CKD, prevention, and delaying progression becomes paramount. Additionally, encouraging patient advocacy through self-care avenues such as home therapy and in-center self-care is important. In order to understand the impact of CKD in a specific area, Visonex has created a CKD platform that allows stratification and risk factor identification to quickly identify, delay CKD progression, and combine with educational opportunities, which may result in a higher incidence of home modality selection through early intervention.
