Dialysis in a Skilled Nursing Facility

Skilled Nursing Facilities are often sought out by family members or patients who have chronic illness or
for focused areas of rehabilitation. These short or long term stay facilities allow residents access to 24-
hour care and needed physical therapy, occupational therapy, dressing changes, intravenous medication
support and more. An ever-increasing percentage of the over 500,000 End Stage Renal Disease Patients
find themselves taking advantage of this advanced care within any given year.

With the recent pandemic, challenges to treating this patient population became increasingly difficult.
Keeping distance, limiting contact, controlling community risk, were all concerns to SNF environments.
This prompted Centers for Medicare Services to issues specific regulations for providing care within the
SNF environment. In turn the benefit for the patients included: increased time in rehab, decreased travel
time, improved quality outcomes with no meal missed, not to mention quality of life improvements such
as staying indoors on cold snowy days.

Visonex’s Electronic Health Record, Clarity, accommodates SNF documentation. Understanding
documentation requirements, however, is just the beginning. Connecting patient records across
multiple providers including Acute Care environments (hospitals) and across SNF Electronic Health
Records, decreasing the need to shuffle paper, file transfer sheets that we hope someone is reading. Is
your facility looking at launching a Skilled Nursing Facility model? How may this impact your staffing,
move you toward home therapies and impact your revenue? Visonex can assist with this information
and more through our SNF Consulting Services. Learn more by contacting us at [email protected]
